Heartgard $5 Rebate with a 12 Dose Purchase

If you purchase 12 doses of Heartgard heartworm pet medicine you are eligible for a $5.00 rebate from the manufacturer.  The Heartgard must be purchased from your vet and your receipt must show the Heartgard product.  A separate rebate form must be submitted with every 12 doses that you purchase, but there does not appear to be a limit on the number of rebates that you can submit.
