To receive your $5.00 rebate by mail, cut out the original UPC code from a package of Colace Capsules 10s/30s/60s or Peri-Colace Tablets 10s/30s/60s.  Include original cash register receipt with the purchase price paid and date circled. Retailer name must be printed on the receipt for verification. Send the original completed mail-in Certificate.
